When an individual walks inside our doors, hope is often the last thing they are thinking about. At Unison Health, hope is who we are. It’s what we do. Unison Health makes it easy for individuals in our community to access integrated behavioral health services and take a step closer to finding hope.

1. Call intake to schedule an individual diagnostic assessment:

  • The intake phone number is 419-936-7525 expect to spend 5-10 minutes on this call

OR

  • Walk-in assessments are available at all three of Unison Health's convenient locations

2. Here is the information you will be asked to provide for you or your child

  • Name, address, phone number (Guardian's name if you are calling for a child)
  • Date of birth, social security number, insurance information 
  • Name of the person/organization who referred you to Unison Health
  • Reason you are seeking services

3. For all assessments please bring:

  • Photo I.D
  • Medical insurance card
  • Proof of income
  • Name of primary care physician

The assessment involves a confidential meeting with a clinical professional to arrive at a diagnosis and plan a course of treatment. This initial meeting generally lasts about two (2) hours.

4. What I need to bring if the appointment is for a child: 

  • Guardian must accompany the child
  • Certified documentation/verification of guardianship
  • Social security card or birth certificate
  • Medical insurance card 
  • Name of referring physician/clinic/school
