Housing First is a program that serves unhoused individuals and focuses on eliminating barriers that may have gotten in the way of finding and/or maintaining housing in the past. Housing First services anyone within Lucas County corridors that is chronically homeless.
To qualify for Housing First:
• Must be 18 years or older
• Can be single or have a family
• Must be chronically homeless 9 months or longer with verification of homelessness. (Verification must be from a reliable source, i.e. doctor, case manager, another mental health agency on their letter head etc.)
• Have a Disability (This is determined by the Housing Impairment Form, in which a diagnosis is needed to complete this document)
• Must complete an SPDAT (Must score in permanent supportive housing to be in Housing First).
Navigator will work with you to:
• Verify disability
• Verify chronicity
• Obtain all needed documents: > ID > Birth Certificate > Social Security Card
• Income (Has to be within 30 days)
• Assist with finding an apartment, house, condo and or mobile home (client’s choice) Stability Coordinator
• Meet with clients 2 times per month (mandatory)
• Maintain and help clients successfully stay housed
• Create a treatment plan and work on goals throughout the year
• Assist with community resources/referrals/linkages etc.
