Care Coordinator

Unison Health is hiring experienced individuals to train as full-time Care Coordinators to work with children/youth with behavioral or developmental disability health challenges and their families.  You will work closely with children/youths, their families and community partners to link the child/youth to needed care and services.  Our service area includes Lucas, Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert and Mercer counties

Qualifications needed:

  • High School Diploma and 3 years’ experience OR Associates or bachelor’s degree and two years’ experience OR master’s degree and one year experience.
  • Experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
  • Expertise in one or more of the following areas: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development. 
  • Must be proficient in the use of computers and systems. 
  • Must possess a valid driver’s license, reliable transportation, be able to be insured through the agency commercial policy and have personal auto insurance.
  • LSW/LPC preferred.

Care Coordinator Supervisor

Unison Health is hiring experienced individuals as full-time Care Coordination Supervisors to lead a team of individuals working with children/youth with behavioral or developmental disability health challenges and their families.   You will ensure that staff are trained, implement the required model of care, provide individualized care based on client need, and link clients to needed services and supports.   Our service area includes Lucas, Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert and Mercer counties

Qualifications needed:

  • Bachelor’s degree in social work, psychology or related field with a master’s degree preferred.
  • At least three years’ experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
  • Expertise in one or more of the following areas: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development. 
  • Must be proficient in the use of computers and software. 
  • Must possess a valid driver’s license, reliable transportation, be able to be insured through the agency commercial policy and have personal auto insurance.
  • Prior supervisory experience preferred.
  • LSW, LPC, LISW or LPCC required.

 Now Hiring Crisis Stabilization Unit Positions

 

Individuals will work in a brand new crisis center located in Bowling Green Ohio. These exciting opportunities will allow staff to work collaboratively to implement crisis care best practices.  Staff will receive extensive and ongoing training as well as support from supervisors that are available 24-7.

Positions include:

  • Stabilization Therapist (Bachelor degree in Social Work or Master's in Counseling)
  • Stabilization Nurse (Associate or Bachelor in Nursing)
  • Stabilization Qualified Behavioral Health Specialist (Bachelor degree in Social Work, Psychology, Criminal Justice, or related field)
  • Shift differentials for nights and weekends.
